IOP vs PHP vs outpatient therapy: how to choose the right level of care
If you or someone you love is in crisis, call or text 988 or go to the nearest emergency room.
Short answer: IOP usually means several hours of treatment a day, three or more days a week, while you live at home and often keep working or studying part time. PHP runs most of the day, typically five days a week, with closer monitoring and more psychiatric care. A clinician can recommend the right level after an assessment.
Outpatient therapy, intensive outpatient programs (IOP) and partial hospitalization programs (PHP) are steps on the same ladder. The right step depends on how much support someone needs each week and whether they can stay safe and functioning at home.
What is outpatient therapy?
- Usually one session a week, sometimes more
- Best for people who are mostly managing daily life and want support, skills and change
- Can include medication management with a prescriber
What is an intensive outpatient program (IOP)?
- Several hours a day, typically three or more days a week
- Mostly group therapy, plus individual sessions and sometimes family sessions
- Clients live at home and can often keep working or attending school part time
- Often used when weekly therapy is not enough, or as a step down after PHP or a hospital stay
What is a partial hospitalization program (PHP)?
- Most of the day, typically five days a week
- More structure, closer monitoring and more frequent psychiatric care than IOP
- Clients go home at night
- Often used to avoid a hospital stay or right after one
How do you choose between IOP and PHP?
Consider:
- Safety: any risk of harm needs a professional assessment right away
- Function: is the person able to work, attend school or care for themselves?
- Symptoms: are they getting worse despite weekly therapy?
- Support at home: is there a safe, supportive place to return to each day?
- Practical needs: schedule, transportation, virtual options and insurance
A clinician can recommend a level of care after an assessment. Many programs do a free phone screening.
Are IOP and PHP available online?
Many IOPs, and some PHPs, are offered by video. Virtual programs can help people in rural areas or with busy schedules. They still need to be licensed and to have a plan for emergencies.
Does insurance cover IOP and PHP?
Most commercial plans and many Medicaid plans cover IOP and PHP when they are medically necessary, usually with prior authorization. Ask the program to check your benefits before you start.
What should you ask an IOP or PHP program?
- What conditions and age groups do you treat?
- What does a typical day look like?
- How do you handle a crisis outside program hours?
- Do you offer virtual or evening tracks?
- What happens when I finish the program?
Frequently asked questions
How long does IOP last?
Many programs run several weeks, adjusted to progress.
Can I work while in IOP?
Often yes, especially with evening tracks. PHP usually requires taking time off.
Is IOP the same as rehab?
Not exactly. Some IOPs treat substance use, but many focus on depression, anxiety and other mental health conditions.
